Qualifications
Required:
- Nursing Degree
- Minimum 2 to 5 years clinical experience with pediatrics, experience in rehabilitation
- Ability to safely and accurately assess patients for acute and long term care pediatric rehabilitation admissions
- Experience interacting with families, health care teams, agencies, case management units and other departments within hospital and or healthcare settings
- Shift working, 8:30 - 4:30pm
- Full Time
- Day Shift
- Travel Required: Up to 75% weekly (including NY and PA)
- Review, cross check and document either inpatient or long term care referral for candidacy and guide next steps in coordination with the referring entity
- Provide friendly and timely communication with internal and external stakeholders and family members regarding intake process, required and outstanding documentation and bridge introductions to appropriate CSH team members and resources
- Electronically document relevant and essential case specific notes and information for each referral to ensure transparent and efficient inter and intra departmental communication
- Proactive and reactive contact with referring hospitals, agencies and other personal to obtain, cross check and document vital information and prevent unnecessary delays or gaps in care
- Ensure that all necessary information is obtained and provided to the proper party
- Travel required
- Other work duties and assignments may be disseminated as needed
